The application period for the 2015-2016 Financial Aid Hockey Scholarship is now open.

Eligibility

Who is eligible to apply for Financial Aid from MaxInMotion?

Youth hockey players ages 5-17.
Players currently registered to participate in Arizona with an accredited association for the 2015-2016 hockey season.
Players with a demonstrated need for financial assistance.

Timeline

Application Period           June 15 – July 10, 2015

Review Period                July 11 – August 14, 2015

Notification of Status      By August 31, 2015

Funds Distribution           By September 30, 2015

Required Application Documents

1.  Completed 2015-2016 MaxInMotion Player Application - available online by end of this week at http://www.maxinmotion.org/apply

2.  Copy of 2014 W-2 Tax Form(s) used to file your 2014 Tax Return (please back out SSN/Tax ID #'s)

In order for an application to be considered, ALL of the required documents listed above must be completed and submitted no later than July 10, 2015.  Photos/jpegs of documents will NOT be accepted.  Please send files as recognized document formats (PDF/Word):
