It is that time of year again when we offer baseline concussion testing.  Our partner is again Banner.  This is not mandatory but recommended.

DATES

Friday, August 11th -  4PM, 5PM, or 6PM
Friday, August 18th - 4PM, 5PM, or 6PM

Players 12 and Up

Testing will be held in groups of 22 at Banner Desert Medical Center.  The cost is $15 per player.  This registration will secure your spot.
 

Players 11 and Under

The baseline testing for players 11 and younger must be individually proctored and not included in this AHU schedule/D.  If you would like to have concussion testing, please contact Brad Meacham by phone at 602-839-7383, or by email at bradley.meacham@bannerhealth.com.  

Reminders

• DO NOT BE LATE! Please plan to arrive at Banner Desert 15 min before your scheduled testing time to allow for parking and navigating the hospital. They will be unable to bring your player into the testing room after the testing session has begun at the top of the hour. If you arrive after the test starts for the group, you will have to attempt to join a later group where there's no guarantee there will be room to accommodate you.

• Attached you will find the consent form; please make sure to print and sign this ahead of time. Players without this form signed, will not be allowed to test.
   
• Tests will be conducted at Banner Desert Medical Center: 1400 S. Dobson Rd, Mesa. See attached driving and parking instructions.
